How to Apply for a Philippine Passport?
Ito ang mga bagay na kailangan ninyo para makakuha ng Philippine passport:
1. NSO Birth Certificate
2. Petition for Dual Citizenship form
3. Issuance of Identification Certificate form
4. Three (3) 2”x2” photographs showing the front, left side and right side views
5. Valid ID
Sundin ang mga sumusunod para sa pag apply ng Philippine Passport:
- Present birth certificate. Present your NSO Birth Certificate that is issued or authenticated by the National Statistics Office. If you don’t have any records in the National Statistics Office, you must present a Certificate of Non-Availability of Birth Record from the NSO. You also need to present a secondary documentary proof of being a former natural-born Filipino citizen such as a baptismal certificate, marriage certificate of your parents, or an old Philippine passport.
- Accomplish forms. Completely fill out the Petition for Dual Citizenship form and the Issuance of Identification Certificate form. Attach three (3) 2”x2” photographs showing your front, left side and right side views.
- Submit form. Submit your completely filled out Petition for Dual Citizenship form to the Philippine Consulate / Embassy together with the photos, birth certificate from NSO, and your valid ID.
- Pay processing fee. You need to pay a processing fee of US$ 50.00. After paying the processing fee, you are scheduled to take your oath of allegiance before a consular officer. Take your oath when your schedule is due.
- Secure copy. The officer gives you an original copy of your notarized oath of allegiance together with your Order of Approval that is issued by the Philippine Consulate General.
- Forward documents. The Philippine Consulate General forwards your petition, oath, order of approval, and other supporting documents to the Bureau of Immigration in Manila for issuance of your Identification Certificate.
- Issuance of Identification Certificate. The Bureau of Immigration issues your Identification Certificate (IC) and forwards it to you through the Philippine Consulate General.

Warning:
- If you are married and you wish to use your married name, you must submit a copy of your marriage certificate when you submit your Petition for Dual Citizenship form to the Philippine Consulate / Embassy.
